I have been promoting to others in our community to complete their form, but I do not know how to answer their questions when they ask how can they get their initial form? I tried to use the web page to locate our assistance center and all it shows is a bubble in the middle of the river, no contact name, address or phone number. We are a county seat community – City Hall, the chamber nor the library knows where to find forms or how to help our constituents.
